In Europe we are bought up to eat with a knife AND fork. Food table manners are instilled from an early age. At least they were in my day. This calls for no swapping whatsoever. My French BF still does this the proper way. Always. Wherever we are. I am a little more sloveny and have become accostomed to the American 'just use the fork in your main hand' way. Shame on me. Particularly as I have evolved to be able to cut through just about anything using the edge of my fork.
